Dumbo's ears
There are many causes of "abnormal ear shapes" at least 135 causes ranging from 2p21 deletion syndrome to Otospondylomegaepiphyseal dysplasia. In particular there are numerous rare eponymous syndromes: Galloway syndrome, De Barsy Syndrome(A rare inherited disorder characterized by loose, inelastic skin, involuntary limb movements, and other abnormalities) and Say-Barber-Miller syndrome. It would take too long to discount all of these so I'll take my conclusion from the WikiQuestion "Why is Dumbo's ears are big? (sic) - He was born that way. Not everyone is perfect."

An unusual case this time, I was unable to see the patients in question as they have consistently been unable to attend any appointments. They claim that their occupation does not leave much time for such things and instead its "off to work we go".
Instead, I have used the collateral information from their carer, Ms White to inform my diagnosis.

It is a dark winter's night at my surgery and the last appointment of the evening. The clinic is deserted and cost-saving measures have meant that only a single flickering light remains on. Suddenly, creeping from the shadows of the waiting room, a dark figure emerges. Dressed almost totally in a form-fitting reinforced suit with his head covered in a frightening mask I can make out the outline of a man - or possibly something more supernatural. At first he says nothing, then quietly, somewhere between a whisper and a threat, his voice rasps "Doctor, I have an itch..."

For reasons of National Security, ‘Mr Smith’ would not give me his real name. However, he informed me that he was “On Her Majesty’s Secret Service”.
He was of slim build, blue-grey eyes, a “cruel” mouth and short, black hair. There was a faint scar of the Cyrillic letter “?” on his hand – which he informed me came from Russia “with love”. On the surface, he appears to be a healthy, attractive man.

Our own Dr Fairytale pays a home visit to his celebrity client in Wonderland for a diagnosis. Here's his medical report -
Multiple Drug Use
Despite her protestations it is clear that Alice has a drug problem. Given
any opportunity she ingests potions, wafers and mushrooms without
concern for her personal safety. She reports bizarre variation in her height,
changes in her perspective, loses track of time and space - even her own
identity. She also refers on a number of occasions to a hookah-smoking
caterpillar which suggests that her social network is that of drug users.
More worryingly, Alice displays a total lack of insight into her problem
and has created a fantasy to justify her drug-induced hallucinations.
